Services Under the HCS Waiver

Host Home / Companion Care
A home-like alternative to group home living, providing habilitation services in a caring family environment.
Day Habilitation/ISS
Facility-based programs focused on self-help, socialization, and adaptive skills development for up to six hours daily.
Nursing Services
Health monitoring, medication administration, and referrals led by our licensed nursing staff.
Behavioral Support
Professional interventions by certified behavioral analysts for behaviors interfering with community participation.
Supported Employment
Assistance finding and sustaining meaningful paid employment opportunities in the community.
Respite Care
Short-term relief services for unpaid caregivers, ensuring continuity of care while giving families breaks.
Physical Therapy
Skilled treatment to improve range of motion, physical conditioning, and mobility for daily independence.
Dental Care
Available to individuals age 21 and older. Up to $2,000 per IPC year is available for emergency, preventive, and therapeutic dental services.
Speech Therapy
Corrective and rehabilitative treatment for individuals with speech, hearing, or language disorders.
Dietary Services
Licensed dietitian assessments, personalized nutrition plans, and staff training for proper dietary management.
Adaptive Aids & Home Modifications
Devices, appliances, and home modifications for daily living, safety, and environmental accessibility.

Who Can Receive HCS Services?
The HCS Program serves individuals with intellectual and developmental disabilities who meet certain criteria. Eligible participants can choose their service provider and may change providers at any time.
- Diagnosis of intellectual disability or related condition
- Mild to severe deficits in adaptive behavior
- Eligible for Medicaid benefits
- Not enrolled in another Medicaid waiver program
- Desire to live in and participate in the community
